I can provide game saves, my save isn't very old due to the bug and not really being able to play, but I'll add that this does happen on all saves for me, even new ones. I'll also add that I play the sims with autonomy on full, with selected sim autonomy OFF.
Just wanted to update this post after the kind people in this thread helping me, it does weirdly seem to be if you have children/ parents. So maybe a connection with the parenting pack? I'm not sure besides it really only happens if your sim has parents.
