I think I may have also experienced something similar, but it happened so quickly that I wasn't too sure.
Basically, I made a male adult and a female adult and gave them a teenage daughter, but I made the romantic relationship between the two adults "Strained" and the family dynamic "Difficult", I also made the family dynamic between the father and daughter "Difficult". Immediately upon spawning the family, the father was in a flirty mood and had some sort of romantic action happening, but while the action was displayed as a 'group conversation' between both the mother and the daughter, he was completely facing the daughter when it happened and the action resulted in a Friendship relationship decrease between the two, with the mother seeming not to care too much. Not sure what happened there.
Not sure if attaching my save file would be much help as this happened quite some time ago (and hasn't happened since), but thought I'd share anyway since this seems to still be an issue for some.
