I'm having this issue also, though I cannot rebirth at all, not even once. Uninstalled all mods, uninstalled Sims. Deleted Sims install and document folders from C:/SimsInstalledHere install and C:Users/Documents. I can replicate the issue by:
Create a new game.
Create random traits/name to leave CAS and start game (have tried multiple character options, genders, orientations, traits/etc.).
Move sim into lot (Have tried multiple combinations of lots/types of lots).
Cheat to complete the first five Soul's Journey by using aspirations.complete_current_milestone five times.
Open CAS using cas.fulleditmodeon to transform sim into a ghost (have tried multiple ghost types).
Cheat to complete the remaining three Soul's Journey by again, using aspirations.complete_current_milestone three final times.
Travel to the Bog and click Rebirth.
Sim swims over to the water and asks if I want to rebirth, and I click yes.
Sim disappears with nothing else.
If I go to manage worlds, the sim household is gone (if only one sim).
If there is multiple sims, the camera will just stay in the Bog where it asked me if I want to rebirth. The only way to leave is by clicking on another Sim in the household and change control to that Sim, or go to manage worlds and pick another household. If I take control of another sim in my current household, there is a baby with the my Sims name, and "rebirth" trait that I can add via "MCCC add to family." It does not take me to a "manage household" or "Create a Sim" screen whatsoever and the baby is just orphaned without a family in the middle of my yard. Without MCCC, the child is stuck as a literal orphan with other unplayed households as a townie.
With MCCC installed I can get the following error in the mc_lastexception.html file:

With TMEX's Better Exceptions I can get the following additional error:

Reason Exception in Sim Timeline: Exception resuming Element (ValueError: list.remove(x): x not in list)
No mods mentioned in the exception.
The issue happens with ZERO mods installed, with only MCCC installed, with only BE installed, and also with both MCCC and BE installed.

I am well aware I have the "modded" value as "true" because I know it's not a mod that's causing it so I've added them back.
@Raulwynn Think this is the trait issue as well.
Does your Sim have any of these
- Untroubled
- Romantic Sage
- Paragon Partner reward trait
- Werewolf
Currently seeing three issues reported with the rebirth
- Crashing
- Sims just swims and disappears (Trait related, please post here: [LD] Rebirth Doesn't Work for Sims with certain traits
- Option greyed out when it shouldn't.
I have tested with brand new sims after completely removing all traits (even CAS traits). I got the swim and disappear part of the bug. It doesn't crash per se, however, it does freeze for 30 or so seconds.
