I had same problem, playing on Xbox with every expansion,game,stuff pack and a few kits…
It was weird because the male sim had a male voice for the first three dates they had together…then he came and knocked on for my sim, I told her to invite him in and his voice had changed to a female sims voice lol from then on every time they saw each other he had a female voice…
luckily when he moved in and I went to CAS to ‘change sim’ and used the slider on the voices they were all male voices like they should be and again out of luck I don’t seem to have the bug where CAS changes are not transferring to live mode…
It was very immersion breaking though and if I’d not been through so many sims with her finding ‘the one’ I don’t think I’d have took the risk of pursuing cause I was worried I would not be able to change his voice and just have to abandon that save lol
I take back what I said in last comment…the changes I made to outfits and voice stayed in live mode for that session…
Ive just loaded the game and it’s saved everything I did apart from CAS changes to the male sim I moved in…now has original outfits and female voice again…guess I am abandoning this save after all…so sick of these stupid bugs ruining the game, just came straight off when I realised…
