As long the changes happen in your favorite (hearted) households they work correctly. It works too as long 1 household is from the favorite tab(My Household) and the other in the other household tab. If this happens to two unfavorited households nothing happen.

 

So I guess @flauschtrud is right and this is because how the game tread this tabs differently.

    @daikoyu Yes, it depends a lot on playstyle. I usually only play with one household favorited, so the impact is bigger.

    The game regularly deletes relationships for performance reasons, but spares relationships from played households, this is what I suspect interferes with the feature. Although that does not explain why the break ups don't work either.
