All packs released until pre-Life&Death patch.
No Mods. New Save.
Created a random male sim in CAS. Randomized name and traits. Did not touch outfits.
Moved him in. Had him take a shower. No undies.
Clicked on the wardrobe to enter CAS and plan outfits.
Clicked on each outfit category, didn't edit, didn't add, just viewed each category.
Clicked Done.
Had him take a shower again. Black undies in the shower.
Tested with two different sims so I doubt it's one specific item that's triggering this.

This looks a lot like they broke something with the changes for the new pack. Sorry EA if i am very direct, but maybe a QA-Team would be a good idea before releasing updates or packs. Never experienced so many bugs anywhere else.
And i want to add that this isn't a cheap game.
I can confirm this still happen even without mods and a new save.
Definitely not mod related. I'm on console and this still happens. No mods no CC.
