I don't think saving and loading is broken. I will verify this soon - all I need to do is get into Live Mode, play a bit, don't enter CAS, shower a few times, "Change into Nude" a few times, SAVE AS... and exit and resume. If the bug ain't present then there's nothing wrong with saving or loading the game. I've been saving and loading my game multiple times over the past month and never encountered this because I never enter CAS in Live Mode and that seems to be "a" or "the" trigger.
I think they broke the "EntherBathing" function or something related to the "EnterBathing" function.
This function is used in every instance where sims get nude, including "Change into Nude", taking a shower or skinny dipping or streaking, etc. - any instance where sims are suppose to strip nude.
I believe this assessment is right because it's the only way I see that this could be affecting all sims after "the bug" is triggered.
My M.A.P. uses "EnterBathing" to force removal of the M.A.P. when taking a shower. It works as it's meant to as long as I don't enter CAS in Live Mode before trying to shower, but once I enter CAS, the bug triggers, breaking "EnterBathing" and my M.A.P. stays on while bathing.
This is why resetting "Nude/Bathing" isn't fixing the problem permanently. It's NOT an issue with the underwear.
Here's an excerpt from my ongoing investigation:
"Something is broken somewhere and under certain circumstances - which appear to include simply entering CAS while in Live Mode - it is causing the default underwear to appear on sims when they're supposed to be nude, but it's not only the underwear because it's happening with my [EP08] Mobility Assistance Platform (M.A.P.) too, after entering CAS.
I haven't been able to trigger it yet without first entering CAS but from what I've read, when it breaks, it appears to break for EVERY SIM IN THE GAME as long as the session continues.
It's also possible that if you save after it breaks, you're saving the "bug" too. At least that's the impression I'm getting from the comments in that Bug Report. I'll test this later to verify.
In all the time I've been in Live Mode over the past month, I have not encountered this, until I followed the directions posted by another simmer in order to replicate the "bug". Simply entering CAS once or twice then taking a shower or using the "Change into Nude" interaction... it just breaks the game!"
If you can mod, you can use an "underwear remover" as a band-aid for now.
"Underwear removers" are usually simple CAS Clones/Overrides of the underwear or the "NudeBase" CAS items with modified (removed or added) "DefaultForBodyType[X]" flags. This was confirmed by another simmer.

... was on the mark when they pointed out entering CAS is potentially "THE" trigger.
I'll be back with anything else I'm able to rule out or verify. The amount of troubleshooting I can do here is limited because of how many updates have passed. For e.g. when we had the Glasses distortion bug, I tracked that issue to the EXE. Swapping to the previous EXE broke certain functions but the Glasses weren't breaking. There's a possibility this problem is EXE too but I can't verify as easily because too much breaks when I swap to such an old EXE. I can't even start a New Game.
I haven't been able to pinpoint anything in the Tuning resources that's broken yet, but thousands of resources have changed since Update 10/01/2024 and Update 10/22/2024 so it's a slow pro- actually, I compared bad there. I compared 10/01/2024 with 12/03/2024 when I should have compared 10/01/2024 with 10/22/2024 since the issue was reportedly caused by 10/22/2024! Pfft! That's what happens when you're sleepy.
I don't think the solution is in Tuning though because I don't think anything in Tuning defines "EnterBathing", but I still want to sift through Buffs with a fine tooth comb.
I think I've ruled out Scale Form GFX resources by override all of the 12/03/2024 SGFX resources with the 10/01/2024 SGFX resources. If the problem was in there, the 10/01/2024 override would've solved it - providing everyone is right when they say this started with Update 10/22/2024.
@SejianBut if you reset bathing outfit in live mode, let them change into nude there is no underwear, If they then change to an outfit and enter shower again, there is still no underwear. The issue only returns after restarting the session.

I get that, but I think it's irrelevant. Fixing the Bathing outfit isn't fixing the problem, and that's why it's not permanent when it's supposed to be permanent.
If you fix the bathing outfit then re-enter CAS a couple times and bath again, does the underwear show up again? I haven't done any troubleshooting that involves fixing the Bathing outfit because I don't believe the root of the problem is there. I think that's a symptom of something else. I'll check it out on my end though.
The thing with my M.A.P. mod is relevant because it's coded properly and everything related to "outfit_type_compatibility" is breaking. It's not the underwear and it's not just affecting the Bathing outfit.

That's actually why I can tell you it's not CAS causing it.
Entering CAS changes the "game mode" and that's why entering and exiting CAS makes the "bug" visible.
Try creating a New Game, enter CAS a few times but DO NOT do any action that strips your clothes. No bathing, no "Change into Nude". Don't reset the Bathing outfit. You can even Visit or Travel to another lot. Save As into a new save game, Exit to Main Menu, then Resume, then "Change into Nude". You shouldn't see any underwear and that's because with everything you just did before "Change into Nude", the bug wasn't triggered.
It isn't triggered until "EnterBathing" is triggered.
I can't explain why it isn't showing the first time it triggers. I can't explain why after that first time, if we don't enter CAS and "change the game mode" or save and resume, that we can shower multiple times and "Change into Nude" multiple times and we won't see the underwear.
What I can tell you is if you start a New Game and immediately "EnterBathing" and save and resume you will see the underwear immediately after resuming, but if you do it with CAS and never "EnterBathing", you'll never see the underwear immediately after resuming.
EDIT #1: I probably shouldn't have included "go swimming and change your outfit using Outfits... Pie Menu" because I haven't tested that bit yet to see if changing outfits on the whole triggers it.
EDIT #2: YEP, looks like one of those two triggers it too! I'll be back, I've got a pot on the stove. I'm literally making popcorn right now! LOL.
