I just had this issue, had my teen sim blow out the candles at a birthday party and no traits were added. I reloaded the save and tried using the cake but not having a party, and I did get the option to add traits. So it seems like the parties are bugged, not the cakes.

In my game the bug even appears with cakes. and with all teenage Sims from other households
