I'm having this problem as well. It was tolerable when it was only affecting toddlers and kids as when they aged up to teens and older, the eyes worked properly. Now, it affects older sims who have a longer life span than those two age groups which in turn make it more frustrating (for me). I saw this happen when I took my vampire into cas to modify his work outfit. Now, not only did his occult eyes revert to human eyes, but he no longer has his vampiric features (fangs and eye sockets). He still has his pointed ears and pale skin, but everything else that makes him a vampire is gone.

It seems like I can't play with a good chunk of my saves now as I am primarily an occult player (especially with werewolves and vampires). With this bug, it feels like I can't even play with the saves I want to because then it will get bugged out. I really REALLY hope this gets fixed soon as it was working before the 10/22/24 update and now, it's bugged out. This is about as bad as elder werewolves' eyebrows turning grey and being unable to change them. It's frustrating to say the least. This issue does seem pretty easy to reproduce, compared to the child/toddler occult eyes bug.
Edit: It appears that this bug does not affect teens. I loaded into my save I've been using for the Reaper's Rewards event. In one of my playable families that is all vampires, the adult and young adult vampires bugged while the teens still have their occult eye colors. I have one YA vampire that still has his occult eyes, however, so I don't know if it's a case that I started the save with him as a teen or if it affects the first two sims (almost like the holiday bug that only affected certain sims in households). Whatever the case, my teens appear safe from this bug, but I still hope it gets fixed soon.
Edit 2: I did a little more testing. At least for me, the bug appears when entering cas with the modify career outfit cheat. I took my sims into cas the normal way (and through shift-click cheat) and my vampires were ok. However, when I took my vampires into cas to modify their career outfits, that's when I saw the bug occur (this is including teens as well). I have no idea if this is what's happening for others, but this is what is happening in my game. So I guess for me, as long as my sim doesn't get a job or I don't go into cas to modify their career outfit, then their eyes/features are safe. Sucks that I can't have them work or modify their career outfits, but until this gets fixed, it's the only workaround to keep them as they were...
Edit 3: I forgot to mention that the vampire I screenshotted and shared was one I made yesterday, after the update. I noticed it right away when I took him into cas to modify his work outfit and he had reverted to human eyes. I'm mentioning this as I'm seeing people say that it seems to only affect vampires pre-update over vampires made post-update. As for my two adult sims in my Reaper's Rewards save, they were vampires made pre-update and were affected by the bug while my three teens and one YA (who aged up from a teen) were safe from the bug. The only time their eyes reverted is when I took them into cas to modify their career outfits. I just wanted to point out that the bug does affect post-update vampires as well, just not in the same way(?).

@DracuAlice2017It's not related to modifying career outfits or anything like that - my occult sim is unemployed and all I used was the basic mirror "change appearance" it just doesnt work the eyes revert to human

@kirpilicious Sorry, I meant that my unaffected sims became bugged when putting them in cas to modify outfits. If you confirm their appearance, then they become bugged themselves and their occult eyes/features disappear and won't stick if you put them in regular cas. I noticed that teens and under are not affected by the initial bug, so this seems to only affect YA-elders(?). It's still weird as it seems to only affect certain sims as one save that I had before the update, the parents are affected but their oldest son (who's also a YA) is not (he was aged up before the update). These were vampires made pre-update, so I don't know why some are affected and why others are not.
I initially thought it was job related, but just loaded into a save pre-update. One of my played families has a vampire in it and from map view, she seemed fine. Upon loading into my main family, she got bugged out and she's a stay-at-home mom. So, when I brought up modify career outfit in cas, that was a sure way to get my vampires bugged out. I didn't mean to make it seem as though that's what's causing it. I just meant that by doing that with my unaffected sims, they become bugged.

Didn't get a chance to read all posts yet, but I wondered if problem you may be having was from when you bring an occult sim back into cas, downloaded from gallery or using dresser or mirror; because for some reason, 'link filters' with 'parts to link' (on bottom left corner in CAS,) if not set the way it was before when sim was created, it resets itself again at 'parts to link- "*ALL", so if you click dark form in there, without setting it to what that bottom left area was before first, or at least un-select them all, When you click dark form in cas, it copies all from his normal form to occult form, making you lose vamp eyes skin detail for vamp etc. Noticed this since a while.
Could this be what is doing this bug, or does this only happen in game when aging up & not visiting cas?
I went into my game & my vampire was fine, & I created a new one, also fine in game, both are young adults.
2nd pic was my settings when I saved in cas after making a sim, then went to play in game.
3rd Pic is when I went to cas again with this sim with my copy of him saved to tray, he comes back in with all parts linked, & if I'd have clicked dark form before setting links like before first, it changes his dark form like the normal form version of him.
It does that with my mermaid too. Not sure about other occults, never tried making them yet.
