Steps: How can we find the bug ourselves? In Game Options, under Pack Setting, uncheck the 'Enable Eco Footprint Gameplay' option.
What happens when the bug occurs? When the 'Enable Eco Footprint Gameplay' option is disabled, Sims still get the 'Gross Air' moodlet and have coughing fits when outside.
What do you expect to see? I expect to see Sims not being effected by the Eco Footprint as the option is turned off in gameplay.
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.
Did this issue appear after a specific patch or change you made to your system? No
I have tried disabling mods, turning the option on and off again, restarting the game and repairing the game.
I just had this again in Willow Creek and I toggled the Eco Footprint gameplay on and off in my Settings. I then voted in 4 NAPs to improve the Eco footprint (even though I didn't want to do any of that). That helped and I was just left with annoying sun rays over the lot. It seemed to be all I could do. I added roof panels and a wind turbine too. The NAP inspector started showing up too.
But I'm sure it only started when I lit all the fires and the chimneys produced smoke. I moved to a new lot in San Sequoia and didn't add a chimney or light any fires and so far it hasn't happened.

Fireplaces definitely make the eco footprint more industrial.
But that shouldn't matter if you turned off the eco footprint altogether...
