@sunstarsims Do you have any debug items or enlarged items on the lot? I have had people share their lots before and sometimes found enlarged or otherwise interfering debug items on the lot causing item-placement issues. I have also fund cases where a roof eave was interfering (though not seen due to clipping), or something to do with stairs or other interactive objects being placed with MOO on that later conflict with the function of another area of needed interactive space. Also items placed under stairs or over them can interfere with the ability to use the stairs, but sometimes only for adults (height difference clearance issue) or sometimes only once they try to descend the stairs - they could go up fine. Or visa-verse. Plus other similar quirky stuff. And just to make it more fun occasionally it seems intermittent when something like this kicks in so I am even more confused then. This is why it would be helpful to share your lot for me to troubleshoot in my game.
I'm trying to remember the circumstances were, but I've definitely seen wall-coverings or other items deleting items across the home when others were placed. If memory serves, in that case it's best to have a Sim on the lot (this can help you track what was deleted since at lease some items will either delete to their household or personal inventory - as already mentioned by others). Then I would try adding the item and see if you can track everything that deleted. Then experiment. You can try things like re-placing items with MOO enabled, tweaking placement (and maybe moving certain items to a new location, and see what sticks/works then. You can try re-tracing the rooms involved. Check for things that may be causing a glitch (like the things I mentioned - enlarged items, debug items, roofing overhangs, etc.) Experiment with removing certain things then re-adding them after to see if they then stick. A room can be re-created sometimes to address certain glitches. Occasionally a room or home has become glitchy and this can help that.
Anyway, things like that are amongst what I test on a lot that has these issues, and as I said, if you share the lot to the gallery I can have a look and let you know if I find a solution or workaround.

The replies from @FinalBossNyuma and @stjmk (replies #13-#15) explain more of the details of my bug very well, as FinalBossNyuma seems much better at explaining it than I! I think the weirdest thing is that it is problems that have never occurred before with MOO on. MOO used to give you absolute freedom to the point of being able to make your build absolutely ridiculous, lol, but now there just seems to be so many restrictions that don't make sense.

I will say that I definitely am aware of the shenanigans with the stairs, and enlarging objects/using debug items. Unfortunately the issue seems to happen on any lot, even without any of the aforementioned triggers. But still, maybe there is something I've missed! Thanks so much, like I said I'll upload a lot ASAP 🙂
oh, PS: FinalBossNyuma explained this well, but I'll just say that the problem tends to occur when first building the home, rather than making changes, i.e: changing the wallpaper or moving something still randomly puts items in to my inventory like you said, but that isn't exactly the problem. It's when first placing items that it says "can't intersect" and refuses to place something, rather than randomly deciding a piano in a different room doesn't fit anymore and placing it in the inventory. Hope that makes sense!

I am very much looking forward to trying out one of your lots in my game to see if I can reproduce the issue - and if I see anything that's causing it or a workaround. You're certainly welcome to make a Bug Report, but if it's a bug your report will have more teeth the more people you find that share the issue, and by eliminating if there's a cause rather than a glitch/bug. Troubleshooting these things can go a long way in that process. 🤞
I ought to add that 1) My game version is also outdated, but not by much! I remember a few months back that there was a hotfix for the skintones within the new, revamped CAS - my game is updated to the version that’s immediately before that patch. And 2) While my primary issues with MOO have to do with already-placed items disappearing after the fact due to certain lot editing, I’ve also experienced that inability to place any items to begin with. In my case, though, it had to do with tucking various items underneath a floating staircase.
The desk had originally been underneath these stairs for a very long time, spanning several updates including far before the revamped stairs update. Then I revisited that particular home again & moved the desk in order to do some renovating. I ultimately got rid of the desk, but before doing that I’d noticed that I couldn’t replace it in the exact spot where it’d just been. And later, I tried placing a small laundry nook with washer/dryer & cabinets underneath the stairs, & nothing would place down properly so I scrapped that idea, as well. Then finally, I managed to finagle a treadmill underneath the stairs. I’d postulated, though, that it likely had more to do with the revamped stairs than with MOO, so due to that uncertainty I didn’t mention that in my original comment.
@FinalBossNyumaI found the same thing at one point in patches over the last year or so – that the 'under stairs' options for functional use changed. I had one lot where the stairs were suddenly deleted by a low half wall under the taller edge of them that had previously been fine. I can't remember if there was a game functionality/feature change that I guessed matched up with it at the time, but I think there was. I had to remove the offending wall and redesigned the area a bit to adjust for the changes.
Over the years there have been a number of these changes with a patch... where a new game functionality effected some type of unconventionally-placed (such as MOO or alt-placed) item within a build I made. But some aspects seemed to have loosened up recently in this regard – particularly with regards to Sim-interaction limitations with some of these items.
I would recommend that you fully update your game, as if your game is not up to date there may be one or more glitch(es) that was/were fixed that may or may not have been listed in patch notes but that could be affecting your game. I would also recommend that you do a game repair afterward, and clear out your cache in case anything glitchy is snagged up in the game cache also. Altogether it would be a housecleaning of sorts. You could go a step further afterward and start a fresh game-save and see if the issues show up there.
