Steps: How can we find the bug ourselves? A child sim dying from anything other then death by fire or drowning. Example: freezing to death, overheating from sauna, death by puffer fish, death from overheating outside.
What happens when the bug occurs? They don't properly die. Grim will reap them and everyone in the household grieves them however they will not turn into a ghost. You get the pop up if you want that sim in your household as a ghost or as a free roaming ghost and they will either stay in the household as a normal sim or if you choose free roaming ghost they just leave the household.
What do you expect to see? Them turning into a ghost—either a free roaming ghost or a playable ghost.
Have you installed any customization with the game, e.g. Custom Content or Mods? Never used.
