I have a similar issue. My sim has a fear of death, but he doesn't even have the option to talk about his fear with anyone, it just doesn't show. I've tried several elderly sims and also a ghost, with none there is the option, although the want is there to discuss his fear. Same with the grim reaper, there is no option to give him the flower.

I am having this issue after the latest update, as well. I believe it's actually all of the fears, but certainly the death one.

The thought bubble you’re hovering over describes what you need to do to get rid of the fear - Click on a Ghost or Elderly Sim or obtain a Death Flower and give it to the Grim Reaper.

I am having this issue after the latest update, as well. I believe it's actually all of the fears, but certainly the death one.

by the pictures, you discussed with the blond sim to the far right?
To me, that doesn't look like an elderly sim or a ghost.

Got the same issue in my brand new game save. Discussing other fears such as 'stuck in dead-end job' fulfil, but discussing 'fear of death' does not, and the fear won't go away.

@TheSimsDirect I wish I had seen this sooner, when I played on vanilla after the update, it was still not fulfilling. I did not notice the pattern but now it fulfils sometimes, maybe after doing the interaction 3 or 4 times, but I cannot say it works for all Sims and all the time. I had put the mods back now so I cannot send the save. But I have screenshots. Young adult Sim talking to an Adult Sim. The action is not fulfilling, but she gained a sentiment from talking to her.

I had this issue where it wouldnt fulfill with an Elder (tried multiple sims) but it DID fulfill with a ghost (yay!)

Same here. I'm having this issue with all fears that require you to discuss fears with another sim and having difficulty in general overcoming fears by doing the required tasks (ie sims put out a fire but it will not overcome their fear)
