Steps: How can we find the bug ourselves? Have a parent punish a child by grounding them, then choose the "no phone" option.
What happens when the bug occurs? The punished Sim will constantly be anxious because they are "breaking the grounding" even though they are visibly not touching the phone, nor you as a player touched the phone icon. The punished Sim therefore constantly gets reprimanded by the family, which makes you lose the relationship and responsibility.
What do you expect to see? I expect the punished Sim not to be breaking grounding when they are not doing so.
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.
Did this issue appear after a specific patch or change you made to your system? Neutral/Not Sure

I finally tested all the punishments and can confirm that only "no phone" one gives the tense moodlet by "breaking grounding". Even if they do homework, shower, sleep, they are "breaking grounding" and are constantly tense and reprimanded. I am putting the images of "no phone" punishment, and for a comparison the "no tv" and "no computer". 

This last image was "no TV" punishment, and even though she is sitting there, there is no tense moodlet for "breaking grounding" because she is not actually watching the tv. Thus, the "breaking grounding" for "no phone" should not happen either, if it is not used.
