YA sims spontaneously turn into urn with no grim reaper or death scene

Occasionally (10% - 49%)What is your current game version number? 1.95.207.1030What expansions, game packs, and stuff packs do you have installed? allSteps: How can we find the bug ourselves? move sim into lot, add sim to existing family, wait for welcome wagon or a few days into game. will happen randomly to npc sim in welcome wagon party or sim added to household.What happens when the bug occurs? the affected sim will spontaneously turn into an urn or tombstone with no grim reaper showing up, and no death scene. sims in household receive sad buff.What do you expect to see? sims should not randomly die for seemingly no reason, and if they do die we expect the grim reaper to be involved somehow.Have you installed any customization with the game, e.g. Custom Content or Mods? @CozySalem  Does this only happen on your current save or also if you start a new one for testing ?

This also just happened to me, my YA sims went to the toilet while I was focused on another sim. The only notification of the death was the sad moodlet on the other sim. Urn was in the toilet. Not sure how I would even go about recreating the situation.&nbsp;

@pollylaua Did you check if any setting on MCCC could be responsible for it ? If you have a backup, I would load that. It's really impossible to tell what was the cause and if it was mod related unless you recreate it on a modless game.

I know OP said they haven't used mods but they haven't come back either and for the it was an NPC so yours might be different.
