I've had that happen after birthdays! It especially happens with teen girls! @Simlady36 calles it FTS (fat teen syndrome). Once the Sim travels, he or she will reset at normal weight. A trip through CAS will also correct this.
Remember in TS2 how when a game would load, sometimes the first shot of a fat Sim would appear thin? Then once the graphics were fully loaded, the Sim would be full-size? I think it's like that but in reverse!
