precisely, it should make sense. Your "empathetic" trait can allow you to ask others,
"Is you okay, is you good? 'Cause I wanted to know"

but their relationship with you determines their response.
Best friends/lovers will open up with you. unless they're a the type whose nature is to build walls. you could break their walls, you can intervene.
Friends - Good Friends should be 50/50, ask "Hypothetical" for advice.
*Pause! Perhaps Empathetic should have a flaw within the trait, or maybe an Inverse counterpart trait Judgmental, Sims who open up to you, you tend to be hypercritical. Judgmental shouldn't be a trait that is "Incompatible" with Empathetic.
Strangers/Aquatinted sims will tend to say "It's nothing, *sigh* I'm fine"
and for the negative relationships (even if it's family), Sims's responses just get nastier
Dislike: "Why do you care?/Not your concern"
Enemy: "Shove off!"
Nemesis "..." - you get nothing from them. just a disgusted look, maybe some daggers.
