I wouldnt mind mute or deaf sims. This would require a sign language skill. Also I could complete my robot sim with that.
Only one arm/hand might be difficult because most basegame actions are animated with two working arms/hands.
I think a wheelchair concept could work too. Maybe with stairs elevators. Would give rise to accident concepts that might hospitals and doctors useful.

Now for the things I wouldnt want in my game.
Seriously physically deformed sims
Mentally challenged sims that essentially behave like strangerville zombies
Anything that involves blood and vomit
