Canonically in the Sims 4 timeline Don Lothario is supposed to be in a romantic relationship with Don Lothario, they just didn't set it up in game, it's probably inferred he sleeps in the same room as Katrina.
He did have his own bed in the old house and he doesn't have a romantic relationship with anyone to start with so I find it strange...
