Doctor Career - NPC Coworkers Don't Work

What happens when the bug occurs? NPC Coworker Sims in this career do absolutely nothing at work. The receptionist doesn't greet anyone and patients just as stand in front of the front desk. The Nurse chats to a single patient over and over all day and sometimes gets into group conversations with other coworkers and or patients also does nothing. Orderly usually just chats to the Nurse Sim. Sometimes they pickup garbage, mop puddles or make beds. The Doctor usually occupies the chemistry station object or sits and uses a computer.
What do you expect to see? Now this could be by design and that would suck but I really would expect Sims to perform their jobs at work. The animations exist, the programming exists so I really don't understand why Sims are so brain dead while on the active career lot for Doctors. Receptionist - Greets patients, chats on phone, uses computer and also assigns beds to new patients. Nurse - Feeds Patients, Examines Patients. Orderly - Makes Beds, Cleans objects/messes Doctor - We should always be able to transfer cases to the other Doctors. We should see them examine patients and run tests and perform surgeries. I feel like the Scientist Career handles Coworkers much better and I really feel like Doctor Career can and should be able to do the same. Why is my Sim expected to run the entire hospital?
Have you installed any customization with the game, e.g. Custom Content or Mods? On console - never used.
Did this issue appear after a specific patch or change you made to your system? Neutral/Not Sure
I really hope this is not by design and that something just broke along the way.
