Internal files containing incorrect information against the games in game descriptions and details, as the average user might not notice (probably only modders) or at least might not notice for a long while (which results to "waiting for additional reports").
I recently posted a bug report concerning a pop-up not showing up where it should. Thing is, due to its nature unless you checked game files you wouldn't even know this feature exists in the first place (and sure enough, it's now marked as "Awaiting Additional Reports").
This is honestly so maddening X /
