Child Sim dying or being taken by social services

My child sim keeps being "rescued" and taken by child services for neglect and/or dying (grim reaper message pops up with the option to make them a ghost - not that this works because they always disappear with CPS) then being taken away.

My sim is a B grade student and all needs are met. I have tried restarting my game, pressing "repair" to fix the problem and using testingcheatsenabled true to turn off accidental death and reset my sim but nothing has worked so far. I only bought Sims 4  Life and Death a couple of days ago and have never experienced this issue before (I have heard others have experienced this without the pack).

I have heard the only way to fix this is to age the child into a teen but I was planning on playing Sims 4 Adventure Awaits with this Sim so I would rather them stay a child...  if anyone knows how to fix this, I would really appreciate help!
