@tc145It looks like a weird case of acne... that refuses to go away.
Crinrict had a problem with adult having acne and commented in an older bug report that by turning it off ( HSY pack ) / exiting / then returning it cleared.
In their case they turned it back on with no further issue.
I would start there I guess?
Failing that to help, I would do the old standby and delete thumbcache ( with acne off ) and see if it clears.

To get rid of acne on adult Sims, while in live mode, open Game Options, go to pack settings, scroll to High School Years, turn off the acne option, and apply the new settings. 

Disease / life affects are not usually changed by changing the sim look in CAS. They are attached to the sim itself. I 'believe' if that is acne, it counts as a disease type thing and sticks.

Oh damn, I didn't know you could turn Acne off, LOL. Thanks for your help.
