Seems like everyone's games have exciting deaths. Mine can be struck by lightning, fix electrical equipment in the pouring rain and nothing happens, no one dies from laughing too much or being too exc...
Other than a mod blocking it, my guess is that you are too careful. (Like me). Also, autonomously they (The Sims) will prevent a death. Even with autonomy shut off, though that gives a higher chance of a death occurring. I've seen many NPC's go to a mirror to calm down. (For example).
For an exciting death (and kinda sad) Here you go:

ohhh I felt sorry for her!
Oh dear, I think I am not cut out to have my sims die! hahahahaha
There were several ways this could have been stopped. But like I said, I let it play through. It was right after a patch update and I realised I needed to update a mod or two. Didn't save the game. I've never seen a fire death so when it started, let it play out.
Kat Prowler is someone I dl'ed off the gallery. Spent all day tracking down most of her CC stuff. Anyways, she hit it off with George Cahill, (They are both 'dastardly', so have alot in comon, haha!) so I think they may end up getting married eventually? lol!

-Could have had George come in and extinguish the fire early. I actually cancelled his autonomous reaction early to put her out and the fire. -Could have called the fire department -Could have had Kat try to put herself out. -Too late at this point, had fire alarms in, but forgot to put in a fire suppression system. DOH!
One thing I'm curious about now though is if someone is on fire, and there is a pool or pond or even a swimable river, lake, or ocean, can they run into the water to put themselves out?
