I think Death By Fear will come, it's just that Paranormal's only a Stuff Pack and they didn't have the budget for that particular Death Animation. I mean, look at all the other brand new stuff they did this time.
Death By Fear obviously happens when your characterrs are Very Frightened for too long, but it would require to be a Free Patch coming with the Base Game because it's basic gameplay. And the guys who do work on the free base game get the lowest budget, and they're working on Babies as well as programming Fear into all the previous packs, you know.
