I am SOOOOOO sick of everybody trying to be so politically correct about disabilities that they just don't bother with them at all.
Some disabilities are easier to do. Physical problems like being wheelchair bound (you can't sit down, you can't use stairs, you might need to buy special machines to wash and toilet, interactions might involve other characters helping you) or asthma and similar (excercise becomes difficult) are easy to program. Epilepsy is a new animation, despite its range of severity, despite there being more to it than that IRL. Perhaps taking medication would become an option and help, but Medication has side effects and limits to how useful it is. Hair falls out.
What'd be tougher is making Sims BEHAVE as if they have disabilities - that's personality programming. Especially mental disorders - depression is more than just 'sad', perhaps it would be impossible to stop being 'very sad', only it's more than that too. Autism is more than being shy; some Autistics are outgoing and gregarious, but that doesn't stop them having lousy social skills - plus a million other things that affect them. Schizophrenia might require thought because there's such a wide severity of it, and it's not like your mates can hear the voices speaking just out of distance. ADD, ADHD, OCD... how would they be made different than just chucking in a few 'traits'? Bi-Polar - well, that's a doozy.
And that's not to say people wouldn't be offended. How would people feel about including the tragedy that is Alschimers, and other kinds of Dementia, where eventually the sufferers forget their friends, their family, and even how to tie a shoe? But what if they're left out? Some people would just be on about EA making money off disabilities if you pay for it and on and on about forcing it or getting it wrong if it was free. Indeed, the devs could TOTALLY mess it up and offend lots of people.
Sims 4 would have to simplify things, but it's working out what devs can and cannot do with the game engine itself that's the tough part.
