I normally leave the pre-mades alone with their clothes, hair and makeup. If one does marry into one of my families then I will give them a slight make over to reflect their new life (new outfit or hair style). I never mess with facial or bone structure. That's just making an entirely new sim to me and if that's what I want then I will just make that sim in CAS.
