I leave premades alone, unless I marry them into my family. Then it's usually minor changes, like giving Cassandra Goth a more adult hairstyle when she grows into an adult, though I try to stay true to their existing styles.
The townies that randomly pop up when the game needs a bartender or a veterinarian though... I overhaul them. They aren't "real people" to me, so they get new bodies, new styles, and often new names. Ones that have the same last name usually get combined into a household and given relationships. Ones that are beyond all help get deleted. If I could turn off the game's ability to generate those things, I would.
