Does your Sim already have other lifestyles? I wasn't able to activate the Health Food Nut until I used the potion to remove the 'Energetic' (or whatever it's called) lifestyle. (My Sims always get that 'Energetic' lifestyle because they do yoga every morning, and it's highly irritating, because they get bored reading books. Also, after yoga they are in an 'energetic' mood, even if I had them do focusing or inspring workouts.)
