Houses become empty when the occupants move out or die. It's annoying but I think it's always been like that.
I don't think you can get the grave except if you were present when they passed away. Or you could use a mod.
"Fill empty homes" will fill the residential lots based on the number of bedrooms listed I believe. For example, if you save a house as having no bedrooms, I don't think anyone will move in it.
