I hope we can choose our own title. Otherwise I assume they will follow the traditional hierarchy of
Royalty
- Emperor/empress
- King/queen
- prince/princess
Nobility
- Duke/Duchess
- Marquess/Marchionesse
- Count/Countess (British: Earl)
- Viscount/Vicountess
- Baron/Baroness
As you can see all titles are gendered. I expect with the sims history of inclusivity and breaking gender patterns, they will try to let players choose the title. I am fine with gendered titles or free choice. Only thing I don't want is the 'male is default, use it for everyone' approach. But I don't see the male only approach working for Noble titles.
As for the career , it is mentioned in the official blog post
The Noble Career invites your Sim into a storybook world of courtly intrigue, lavish pageantry, and dramatic power plays. From Noble Newcomer to Kingdom Leader, your Sim can rise through the noble ranks and claim the Throne as Monarch of many Kingdoms
Advance Noble Rankings by earning favor at court, forging powerful alliances, or pursuing bolder paths through clever schemes and strategic marriages. And yes, your Sim’s spouse and children inherit those shiny titles, too.

There are no fixed hours in this career; performance hinges on completing Royal Duties each day.
I hope they do let us pick gendered options.
