But possibly enabled or bugged to appear in the bugged situational outfits the game decides to dress Sims (since CL) and been begging this stop for years, with other situtional game picked outfits. It has borked so many times it's not even funny anymore. ETA: Especially possible if it's mesh was cloned from the towel head cover or other head dress covering (hats) in game...got to learn how to set things not to hold on to previous cloned meshes...and or bhavs on objects. Back to 101 coding for meshes and objects.
