Found it its in CAS. Though I didn't see it in CAS.

It isn't something you can buy or put it on in CAS. It is something tied to an infant.

When you click on an infant and choose baby care, you can carry them in the back carrier. If you click more, you can even set the preferred back carrier for the infant. This is where one of the exclusive contents is.

From the early access reviews it looked like you clicked on your sim when they’re holding infant to access the carrier.
