Is the nurse completely useless in Get to Work or is it just in my game?

I mean in the doctor career. The nurse and the orderly just keep talking to patients. At least the orderly cleans the bed, but the nurse is completely useless. My sim is already the doctor. I thought ...

And they always pick the one patient you need to use to achieve a task so you are blocked from progressing in your career ( except if you use a sneaky mcc delete sim).
