They couldn't die in TS3, nor could pregnant sims.
Kids in ts3 could die from fire, drowning and meteor strike.
Kids in ts2 can die from fire, drowning and satellite
Kids in ts1 can die by hunger (but neglect like starving a kid since 2 results in social services taking them away)

Accidental death = ok
Neglectful death(starving, prolonged disease) = not ok
And yes while pregnant sims I believe cannot die they did have a story line for the Monty family in ts2 where Olivia Monty (Romeos mom) died while pregnant with https://sims.fandom.com/wiki/Unborn_baby_Monty
The mechanic was likely too traumatic for a game they censor (juice not alchol, woohoo not intercourse, goose not grope etc) to allow kids to play
