I refrain from culling the dead sims as to not messing up my family tree in the game, you could use mods if you are not that antimods player to prevent ghost visiting others lot.
I didn't use mods at the time of posting this but have been using mcc since on a fresh save to stop them from disappearing... problem is my thing is like 130 families each with deceased ancestors. Before I had the ancestors killed off through old age there was 197 "played households" so if I don't allow culling, I can't then populate the next gen cause 200 is recommended.

My game culls ghosts all the time (no mods), and they stay in the tree. The only time a Sim comes up with a blank head is when I actually delete the Sim in CAS or Manage Households. Possibly a mod problem? Or is your save older than the patch? If your save is older than the patch, it might be that it's only fixed in new saves. I started playing legacies about a year and a half ago, and I've never had this issue - when did the patch come out?
I've had this issue both in modded and modfree play with new saves. I didn't use mccommand until this happened 3 times in separate recreations of this save file. So far I've tried natural death (aging in just playing other families), then making them die actively on screen so I always get a tombstone then I tried releasing all the spirits right away. I still get holes in the tree.
