Okay I've tested.
My parent sims:
So I made them try for baby, then saved. After that I cheated the labour with MCCC, aged up a kid and took a screenshot. Repeated three times.
It's really the same sim, only her weight was slightly different (and maybe body in general, it's always quite random). Facial features didn't change.
Then I reloaded again and gave mother a few carrots.
A boy turned out different.
After that I reloaded again and deleted the father during 2nd trimester. A girl is just her mum's copy.
