Sex and number of babies is determined at conception. Skin colour (though limited to light/medium/dark or unnatural colours as Baby) at birth.
Not sure if hair/eye colour and hair texture is decided then or once the kid ages to Toddler (when it's first visible). Skin tone could potentially change with aging up too, as the range of tones is more varied for Toddlers.
Might be worth saving before aging the baby to toddler and reloading a few times to see if it changes each time.
