It seems to happen mostly in new builds. That may be why the Blue Velvet works, the stairs existed before the glitch. But in new builds especially if you auto place fencing around a mezzanine and then add stairs, the fence will disappear at the top step, but your sims won’t climb.

You don’t have to change the original lot, just move the stairs if the fencing appears where the top step is supposed to be, sledgehammer that section out and put the stairs back into their original position.
