Real-life illnesses
Big fat no. Like, I have even rejected otherwise good sounding mods just because they included rl sickness. I wouldn't mind if the current illness system had more consequences but there is absolutely no situation I want to see a "your sim has cancer" message ever in my Sims game.
More "trendy" social media influencer tik tok whatever modern tech nonsense
Enough. Don't care if it "reflects real life". It's annoying (both irl and in game), ages poorly, and makes for a boring gameplay. They already make me want to smash their stupid phones. And look... I myself might be bit of a tech-head geek in love with my PC but, bruh, I have zero interest to watch my sims sit in front of a computer while I'm sitting in front of a computer playing a game.
Same ol' generic suburban worlds
I want more Forgotten Hollows a.k.a worlds in which they went "all out" with a theme. The squandered potential of RoM annoys me. Like, yes, it has the magic realm which is gorgeous but you can't live there and the livable world itself basically has nothing magical about it and just looks like your usual quiet woodsy neighborhood (with weird humongous driveways in a game with no cars). Werewolves at least had that grungy aesthetic and some exploration.
RL brands, franchises, celebrities, characters, etc
Basically what @Flikka_Flikka said. I too am fine with unobtrusive parodies and subtle nods (e.g. "Game of Llamas" on TV Season Premiere Day), but I don't want to have my immersion interrupted by ads. That is one reason (among others) why I'll never get Journey To Batuu even though I actually really love Star Wars (the old school stuff especially). That pack is basically just an ad they forcefully pushed through against player wishes.
