Yep, it’s this. They don’t get the moodlet anymore by the time they’re a Teen or older, but the thought bubble still randomly comes up for some reason. Pregnant Sims also get the diaper thought bubble sometimes, but it doesn’t seem likely that’s the reason in OP’s case.
Yes, I checked and all four teens have the Happy Toddler I guess they're just reminiscing of the good old days. LOL
