You can technically use existing Sims, but only if you are willing to separate them from their family tree. I'm keeping my own family tree with lucid chart, so I didn't mind, but it's not ideal if you do care. You can use full edit mode in CAS to make them related to your family again too, but depending on their age it may not be in the same way it was.
You save the Sim to your library, then go out to the main menu and start a new scenario. In CAS, click on the gallery button, choose the Sim you saved, then choose to replace the current Sim. Now your own Sim is in the scenario.
