Not sure about painting, but I've played with the vet career a few times and never remember a third employee being an absolute necessity. The pre-made clinic only has two exam tables anyway, so if you're using that, then there isn't much for a third employee to do. I had my sim and one of the employees do treatments, while the other employee handled cleaning.
