I used to always edit my kids' appearance. I would go into cas.fulleditmode once they became a toddler, make them a YA, tweak them to my liking, and age them back down. I don't do it anymore, it's just not as fun. Then again I'd tweak even if it was a minor thing that bothered me. Now I think any little "imperfection" gives them character instead.
I think genetics as a whole have improved, though. Sure, the "play with genetics" thing generally turns out bad (no chin syndrome etc) but when you actually make them have kids in-game they pretty much always look good.
