š Death is too major an event and too impactful on the chronicle that I'd allow it to happen randomly. Fortunately random deaths are easy to avoid, so I don't face that problem often.
If it happens against all expectation, I either reload or instantly resurrect the "dead" sim and assign them a drawback. What that is depends on the story I'm playing, it can be anything from wearing a bandage for a couple of days to a new occult state.
Just to give an example, in my Star Wars save a toddler was forcefully adopted by an imperial family and my sims are making plans to steal him back. That's heartache and tension enough (since the outcome will depend on my sims' actions, can go either way). Their bed randomly falling on one of the involved characters would add absolutely nothing to the drama, to the contrary, it would feel like comedy.
There are many challenges around that include dice rolls for random deaths, the Oregon Challenge being my favorite in this regard. Those rolls all occur on expected points and therefore make the resulting story feel real. Justified randomness will always be more satisfying for me than wanton randomness.
