Anybody else in the household tends to do the "check toddler" interaction incessantly whether or not they're set to be a caregiver or not. The difference is with the discipline interactions. A sim has to be specifically set to be a caregiver (they'll get "from being a caregiver" whims just from living in the same household even if they haven't actually adopted the toddler as a care dependent) in order to be able to tell the toddler to not make a mess, have a tantrum, etc.
A sim living in the same house has a toddler who they haven't adopted as a care dependent pretty much as all of the responsibilities for the kid, just without any authority.
