If they were born into a family and not made in CAS, I try to come up with a name that sort of "matches" the others. Like if Bob and Betty had a child, I wouldn't name him/her something longer/fancier/with lots of syllables. It'd be simple, like Mary or John, and vice versa. Or something that fits into the story of the parents, like musical Sims naming a child something like Melody.
If I'm making a Sim in CAS, it's pretty similar. Sometimes I start with a name already in mind and design a Sim around that, or sometimes I pick a name that suits their appearance/personality. I try never to use names of people I'm very close to or have strong memories of (good or bad) because it feels a little weird to me, so my Sims often have names that are less common. If I'm really stuck I'll visit a baby name website (or hit the randomize button 80 times until I like one). I'm pretty sure that targeted ads think I'm pregnant since I'm on sites like that for my Sims...not super often, but often enough. :D
