For my CAS/starter sims, I usually randomize their appearance a bit or have an idea of the kind of sim I'm going to make, then I just hit the random button as many times until I get something that's ethnically fitting for a last name. After that the first name can be anything, usually something random, but not too exotic.
For sims born in game, it really depends on the story. For example, one generation a sim married a townie and took on an Irish last name, so I named all of their children with Irish names from a baby name website. Or when an earlier generation married a scientist with a Hispanic last name, their human children got Latino first names and their alien children got exotic names. Or later on I had a sim named Hope who married Hugo from Windenburg, and so I named all 6 of their children starting with the letter H so they all had the initials H.V. I'm weird. lol
