Ultimately I found the Medicine career frustrating and repetitive, aside from a few good animation gags, and I never got to max level as I have done with Science (on more than one character). Provides some great toys for your house too.
For the non-active careers, I go with Writer and Tech guru most often. You can just put a PC down in the living room, next to the fireplace and stereo, and tell your Sim to go Program/Write. Your family is able to chat to them while they work and their needs stay up. You're not banishing them to the Kitchen or the Easel and you don't need to micromanage much. This is especially true of Tech Guru since the basic salary is decent, I only ever "practice programming" rather than having to queue up book after book.
Astronaut is cool, but you have to work on two quite disparate skills (logic, fitness) though i still chose it often.
Business and Sports require charisma, which isn't easy to level up. Worse, personal trainer requires you to go to work in a confident moodlet which is not as easy to arrange as Inspired (shower, browse art) , focused (chess board, pc) or energised (shower, coffee, workout). The daily task, mentoring, takes ages, and your sim can't do anything else at the same time - they're not working on their own fitness or building charisma, the other prime requisite!
Charisma is easy to level up if you're not opposed to having your sims talk to other sims a lot it goes up way quicker if you're using the charisma interactions then it does to have them practice in front of the mirror. Easiest and quickest way to get a Confident moodlet is to have your Sim brush their teeth it makes them Confident very quickly and lasts for four hours!
