Why is this hand always popping up in their thought bubbles? (photo)

They used to only show that when they were leaving but I am seeing that a lot. Also a file tray, and a pillow. They seem to just use them randomly now.

Some of the thought bubble images get stuck (it's on top of the actual image if you look closely). I've had repeats of dog, frog etc. Not sure if this bug has been reported?

Maybe she's learning kung-fu.

She's saying "If you touch my leg I'll karate chop you in the carotid".

It's a bug they have not fixed. You can see the thought bubble of the hand overlaying the speech bubble where she is talking about a vacuum cleaner - the overlapping thought bubbles are random but get repetitive and spread to sims all over the town.

I’ve had this too, and it wouldn’t go away until I restarted the game. Mine at one point was engagement rings I think? It started happening to me a couple months ago. Doesn’t happen too much, but it is annoying.
